Does Cigna Cover Wegovy in 2026? Coverage, Cost & Approval Process

Yes, many Cigna plans are accepting prior authorization for Wegovy in 2026. Cigna's Express Scripts pharmacy division continues to place Wegovy and Zepbound on its “big” formularies, and Cigna's EnCircleRx and EnGuide programs limit eligible members' weight-loss GLP-1 copay to $200/month or lower. A BMI of 30 or more (or 27 or more if the applicant has a weight-related condition) and documentation of a lifestyle program is also required, and coverage is limited where the employer provided the weight-management benefit. Wegovy's heart protection approval provides an additional pathway for cardiac patients to receive treatment.

Cigna's Open-Formulary Approach to Wegovy

CVS Caremark went exclusive with Novo Nordisk but Express Scripts maintained both of the major weight loss GLP-1s and allowed clinical suitability to be the deciding factor. For Wegovy, it's coverage without the hassle of stepping in and out of coverage, as Caremark did, but some Cigna plan designs will still require you to start with Wegovy in step therapy due to its cost position, which can work to your advantage if it's a plan you wish to pursue.

The cost-control layer is what employers buy. EnCircleRx will bundle GLP-1 coverage with clinical management, financial guarantees and provide plan sponsors with a pharmacy arrangement where member out-of-pocket costs for covered weight-loss GLP-1s will be capped at $200 per month or less on participating plans. This is because it was not included in benefits until 2026, making members of a Cigna medical plan more likely to have any weight loss GLP-1 benefit. 

Cigna's Prior Authorization Criteria for Wegovy

The clinical gate is based on FDA labelling. Adults are eligible with a BMI ≥ 30 or ≥ 27, plus 1 weight-related condition, for example, hypertension, type 2 diabetes, dyslipidaemia or OSA. Usually Cigna will need proof of a reduced-calorie diet and exercise regimen, typically for 3 to 6 months, and a baseline weight that is on file. Some plans will cover adolescent coverage (from age 12 and older) for the pediatric indication.

Renewals are based on the standard benchmark of ≥5% weight loss from baseline in the initial authorization period. The parallel route is the FDA's cardiovascular approval in March 2024, which allows prescribers to use Wegovy as a heart-disease treatment in patients aged 18 years and older with known cardiovascular disease and a BMI of 27 or higher, regardless of the weight class. 

What Wegovy Costs on Cigna in 2026

Plan design determines the number; standard brand tiers range from $30 to $150 monthly; EnGuide-participating plans have GLP-1 copay of $200 or less until deductible amounts are reached; and for high deductible members, copays are negotiated ($650 to $900) until deductibles are met. Novo Nordisk's savings card reduces copay for eligible commercial customers by up to $100 per month and combines with most Cigna designs.

If not covered, NovoCare Pharmacy and other big retailers charge around $499 per month, while the federal TrumpRx channel charges near $350 per month as it gradually rolls out through 2026, and the list price is around $1,350 per month. Cigna Medicare members get the $50 monthly federal cap via the cardiovascular route now or the GLP-1 Bridge pilot from July 1, 2026.

Approval Strategy and the Appeal Sequence

Submit complete first time: baseline BMI, weight history, documentation of comorbidities, program notes and cardiac records (when using the cardiovascular route). Cigna usually make their decisions in 72 hours, with 24 decisions being expedited. Schedule a date of expiration and continue to chart weigh-ins for renewal.

Denials go in the normal two bins. Benefit exclusions (employer didn't purchase the category) are sent to HR where EnCircleRx and the $200 copay will be formulated to change just that conversation at renewal. Clinical denials -- respond to documentation, appeal decided in 30 days (72 hours expedited), then independent external review). Completeness of files is very difficult to refute using the objective criteria.
