Medicare Supplement Insurance (Medigap) in Texas is one of the biggest and most competitive markets in the nation. In Texas, there are about 4.4 million Medicare beneficiaries, the second highest number of Medicare beneficiaries in a state after California. The Texas Medigap market consists of carriers who are primarily national giants such as Blue Cross Blue Shield of Texas, Cigna/HealthSpring, Mutual of Omaha, Aetna, Humana and State Farm as well as carriers that operate in the region or offer specialty coverage. The average Medigap Plan G premium in Texas is $163 per month, Plan N is $120 per month and Plan F is $209 per month. Plans G, F and N represent more than 95% of Medigap enrollment in Texas. About 40% of new enrollee selections are for Plan G. Importantly, Cigna will brand their Medicare business over to HealthSpring in 2026, meaning that Medicare beneficiaries that compare plans will see HealthSpring rather than Cigna for Medigap policies. Medical underwriting is permitted outside the Medigap Open Enrollment Period (6 months after Part B begins), which means carriers can refuse to cover you or charge higher premiums if your health history if you don't enroll in the first six months. Texas premiums can differ significantly. The range varies by plan type and the state-wide average for 65 year olds is $39 - $549 per month. In major metro areas, the cost of Plan G is around $130 to $250/month. This difference is due to the different pricing methodologies (community-rated, issue-age-rated, attained-age-rated), geographic factors and carrier strategy.

The coverage for all Medigap policies is standardized at the federal level: Plan G from Carrier A offers the same basic coverage as Plan G from Carrier B. The only variations are rate stability over time, premium, and a customer service.

Top Texas Medigap carriers and costs

Blue Cross Blue Shield of Texas (BCBSTX) is the biggest Medigap carrier in Texas. The carrier has a competitive Plan G rate averaging $166/month and has good customer service ratings. BCBSTX also provides "Select Plans" that have provider networks at reduced premiums (like HMO structure).

Cigna/HealthSpring (rebranded for 2026) has some of the lowest Plan N prices in Texas with an average of $104/month. The change in name to HealthSpring is for all Cigna Medicare business, but does not impact the coverage itself. Note: Cigna has more complaints than the carrier average.

Mutual of Omaha has high customer satisfaction and rate stability ratings. Mutual Of Omaha Plan G in Texas has an average rate of $175-$200 per month. The carrier has a special appeal for the long-term rate stability.

Plan G is offered by Aetna at high premium prices ($180-$210/month average) and with good customer service.

Humana has competitive Plan G rates in Texas covering multiple geographic areas.

State Farm has high customer satisfaction and competitive Plan G rates.

Transamerica is also one of the only carriers in Texas that provides all the 10 standard Medigap plan types. The carrier has competitive rates for Plans K and L (high-deductible cost-sharing plans).

In Texas, Ace Property and Casualty offers the lowest Plan G at $137/month, $26 less than the state average. Competitive Plan N: $99/month, Plan F: $153/month as well.

Wisconsin Physicians Service (WPS) has competitive Plan C rates ($220/month) for those eligible for Medicare prior to 2020.

Plan G: The most popular Texas choice

Plan G is the most popular Medigap plan in Texas, chosen by approximately 40% of new enrollees in 2026. It's the most comprehensive option available to anyone Medicare-eligible after January 1, 2020.

Plan G covers all Medicare-approved costs except the annual Part B deductible ($283 in 2026). After meeting this small deductible, Plan G enrollees typically pay $0 out-of-pocket for covered services, including doctor visits, hospital stays, outpatient care, durable medical equipment, ambulance services, and the first 3 pints of blood.

The 2026 Texas Plan G premium range:

  • $130-$165/month (most affordable carriers, healthy 65-year-old)

  • $165-$200/month (mid-range carriers, average Texan)

  • $200-$250/month (high-tier carriers with rate stability)

High-Deductible Plan G (HDG) is an alternative for budget-conscious enrollees. With a 2026 deductible of $2,950, HDG offers significantly lower premiums but requires paying that deductible before benefits begin. HDG works well for healthy enrollees who rarely use medical services.

Pricing methods affect premium growth:

  • Community-rated: Same premium for all enrollees in a state, regardless of age

  • Issue-age-rated: Premium based on age at enrollment, grows only with inflation

  • Attained-age-rated: Premium based on current age, grows annually (most common in Texas)

Plan N and Plan F alternatives

 Plan N is the second most popular Texas Medigap plan, on average $120 per month statewide. Plan N has smaller office visit copays ($20) and emergency room copays ($50), but the cost of the premiums is higher than Plan G.

Plan N Pros: Monthly premium is $30-$50 less than Plan G. Equal access to a doctor across the country as Plan G.

Plan N deductibles and copays: $20 office $50 ER (if not admitted). Plan N will not pay excess charges for Part B (rare in most states and a factor to consider in Texas with some specialists billing more than the Medicare allowance).

Plan F offers the fullest coverage of all plans (including coverage of the Part B deductible), but can only be used by individuals who are eligible for Medicare prior to January 1, 2020. The highest premium of the popular plan in Texas is Plan F at an average of $209 per month.

The choice of Plan F vs Plan G (for people enrolled before 2020): Plan F appears to be quite a bit of coverage, but is $25-$50 more expensive per month than Plan G. With the $283 Part B deductible (the single difference in coverage), Plan G is typically the more economical option mathematically.

Texas Medigap enrollment rules

The Medigap Open Enrollment Period is critical. You have a 6-month window starting the first day of the month you turn 65 AND are enrolled in Part B. During this period, insurers must sell you any Medigap plan regardless of health history at the best rates available.

Outside this Open Enrollment window, Texas allows medical underwriting. Insurers can review your health history, request medical records, and either deny coverage entirely or charge higher premiums.

Limited guaranteed-issue rights exist in Texas in specific situations:

  • Loss of employer health coverage

  • Medicare Advantage plan termination

  • Moving out of your MA plan's service area

  • The 12-month Medicare Advantage trial right (first time on MA)

Verify discounts. Many Texas carriers offer discounts for non-smokers, married couples, women, electronic payment, and household discounts.
