Three GLP-1 drugs come in pill form. Two are approved for weight loss. One is approved for diabetes only.
Generic name: Oral semaglutide 25 mg. Approved for: Weight loss and cardiovascular risk reduction. FDA approval:December 2025. Dose range: 1.5 mg, 4 mg, 9 mg, 25 mg daily. Weight loss: 13.6% average. Up to 16.6% with full adherence (OASIS 4 trial, 64 weeks).
Timing rules: Yes. Empty stomach, 4 oz of plain water max, 30 minute wait before food or other meds.
Cost: $149/month self pay for starter dose. With savings card, as low as $0 to $25 with commercial insurance.
Storage: Room temperature. No fridge needed.
Wegovy pill produces the most weight loss of any oral GLP-1 right now. The tradeoff is the morning fasting routine.
Generic name: Orforglipron. Approved for: Weight loss. FDA approval: April 2026. Dose range: 0.8 mg, 2.5 mg, 5.5 mg, 9 mg, 14.5 mg, 17.2 mg daily. Weight loss: About 12.4% at the highest dose (ATTAIN 1 trial, 72 weeks).
Timing rules: None. Take any time. With or without food. No water restrictions.
Cost: $149/month self pay for starter dose. Lilly savings card brings it to $25/month. Medicare Part D patients may pay $50/month from July 2026.
Storage: Room temperature.
Foundayo produces slightly less weight loss than Wegovy pill. But it's the easiest GLP-1 pill to take. No special routine needed.
Generic name: Oral semaglutide 7 mg / 14 mg. Approved for: Type 2 diabetes only. Not weight loss. FDA approval:2019. Dose range: 3 mg, 7 mg, 14 mg daily. Weight loss: 3% to 5% (off llabel, not studied for obesity).
Timing rules: Same as Wegovy pill. Empty stomach, small sip of water, 30 minute wait.
Cost: $900 to $1,000/month without insurance. May be covered under diabetes benefits.
Storage: Room temperature.
Rybelsus is the oldest oral GLP-1. The doses are lower than Wegovy pill. Weight loss is modest. It's a reasonable option if you have diabetes and want oral GLP-1 therapy, but not the best choice if weight loss is your main goal.

Most weight loss: Wegovy pill (13-17%).
Easiest to take: Foundayo. No timing rules, no fasting.
Cheapest starter dose: Tie. Both Wegovy pill and Foundayo cost $149/month self pay.
Best insurance coverage: Varies by plan. Ask your pharmacy.
Approved for weight loss: Wegovy pill and Foundayo. Rybelsus is not.
Cardiovascular protection data: Only Wegovy pill. Foundayo doesn't have cardiovascular outcome data yet.
The weight loss from Wegovy pill (13-17%) is close to injectable Wegovy (about 15%). They use the same molecule at different doses.
Injectable tirzepatide (Zepbound) still produces the most weight loss of any GLP-1 drug, around 20-22%. No pill matches that yet.
If you're currently on an injectable and thinking about switching to a pill, that's doable. If you're moving between different drug classes (say from tirzepatide to oral semaglutide), your doctor needs to plan the transition carefully. People who don't want injections. That's the main reason.
Also people who travel a lot. Pills don't need refrigeration or sharps containers.
People who want a lower entry cost. The $149/month self pay starting price is less than most injectable GLP-1 drugs without insurance.
And people new to GLP-1 therapy who want a less intimidating starting point than a needle.
People who want maximum weight loss. Injectable tirzepatide still leads.
People who forget daily pills. A weekly injection is one task. A daily pill is seven.
People who are already on an injectable that works. Switching to a pill for the sake of switching doesn't make sense if your current treatment is producing results. If cost is the concern, there are savings programs for the injectables too.
Amycretin (Novo Nordisk) is a dual GLP-1/amylin pill in phase 2. Early data showed 13% weight loss in 12 weeks. That's faster than any approved pill. Phase 3 results are expected within the next year.
Other small molecule oral GLP-1 drugs are in earlier development. The number of pill options is going to grow.
