The Medicare Savings Program (MSP) is one of the most valuable benefits in the Medicare system for low-income beneficiaries. Run by state Medicaid agencies, MSPs cover Medicare premiums, deductibles, and sometimes coinsurance, potentially saving qualifying beneficiaries over $2,400 annually plus automatically enrolling them in Medicare Extra Help, which adds another $5,700 in annual prescription drug savings. There are four MSP programs (QMB, SLMB, QI, and QDWI), each with different income limits and benefits. The 2026 income limits range from $1,350 single ($1,824 married) for QMB to $1,816 single ($2,455 married) for QI. The 4 Medicare Savings Programs

Each MSP serves a different income range and covers different costs:

1. Qualified Medicare Beneficiary (QMB). The most generous MSP. Covers Medicare Part A and Part B premiums, deductibles, coinsurance, and copayments. Beneficiaries pay nothing for Medicare-covered services from providers who accept QMB.

2. Specified Low-Income Medicare Beneficiary (SLMB). Covers only the Medicare Part B monthly premium of $202.90 in 2026. Beneficiaries still pay for other Medicare costs (deductibles, coinsurance).

3. Qualifying Individual (QI). Also covers only the Part B premium. Unlike SLMB, QI funding is limited and operates on first-come, first-served basis. You must reapply each year. Cannot be combined with Medicaid.

4. Qualified Disabled Working Individual (QDWI). Covers the Medicare Part A premium for disabled people who returned to work and lost premium-free Part A. This program is rarely used due to specific eligibility.

QMB enrollees also receive automatic Extra Help for Medicare Part D, with prescription drug copays capped at $4.90 in 2026. SLMB and QI enrollees pay no more than $5.10 generic / $12.65 brand-name through Extra Help.

The income limits include a $20 general income disregard. SSI counting rules apply, meaning approximately half of work earnings aren't counted.

Resources include checking and savings accounts, stocks, bonds, mutual funds, retirement accounts, and real estate other than your primary home. Resources NOT counted include:

  • Your primary home

  • One vehicle

  • Life insurance policies

  • Household goods and personal effects

  • Burial expenses up to $1,500 per person

  • Burial plots

Important state variations:

  • No asset limit states: Connecticut, Delaware, Louisiana, Maine, Mississippi, Vermont (and several others)

  • Higher asset limit states: California ($130,000 single / $195,000 couple for 2026)

  • Higher income limit states: Some states (Connecticut, Maine, DC) have higher income limits

  • Alaska and Hawaii: Higher federal poverty levels mean higher MSP income limits

What QMB actually covers (the most generous program)

QMB is worth the most because it eliminates virtually all Medicare cost-sharing:

Premium savings:

  • Part B premium: $202.90/month = $2,435 annual savings

  • Part A premium (if applicable): up to $518/month = up to $6,216 annual savings

  • 2026 IRMAA surcharges: N/A for QMB enrollees

Deductible savings:

  • Part B annual deductible: $283

  • Part A deductible: $1,736 per hospital benefit period

Coinsurance savings:

  • 20% Part B coinsurance: capped or eliminated

  • Part A coinsurance days 61-90: $434/day eliminated

  • Part A lifetime reserve days: $868/day eliminated

Additional benefit: Automatic Extra Help enrollment with prescription drug copays capped at $4.90 in 2026. The combined value is conservatively $7,000+ annually for QMB enrollees.

A practical note: federal law prohibits Medicare providers from balance-billing QMB beneficiaries beyond what Medicare and Medicaid combined pay. This protection is sometimes violated. If you're a QMB enrollee, never pay a Medicare provider bill until you confirm with state Medicaid whether you owe anything.

How to apply for a Medicare Savings Program

MSPs are administered by state Medicaid agencies, not by Medicare or SSA directly. To apply:

Step 1: Find your state Medicaid agency. Visit Medicaid.gov and click your state to find contact information.

Step 2: Complete the application. Most states accept online, paper, phone, or in-person applications. Required documents typically include:

  • Social Security cards for you and your spouse

  • Proof of income (pay stubs, Social Security benefit letters, pension statements)

  • Bank statements

  • Proof of residency

  • Medicare card

Step 3: Submit and wait for determination. State Medicaid agencies have specific processing timelines. Most states determine eligibility within 45 days.

Step 4: Re-apply annually for QI. The QI program requires annual reapplication. Priority is given to people who received QI benefits the previous year.

What happens after MSP approval

Once approved for an MSP:

Your benefits start retroactively (in some cases). SLMB and QI entitlement may be retroactive up to three months prior to your application date if you were eligible.

You're automatically enrolled in Extra Help. No separate application needed.

Medicare premiums stop being deducted from Social Security. Your monthly Social Security check increases by the amount the MSP now pays.

You receive notification by mail. State Medicaid notifies you of the start date and what's covered.

You may need to enroll in a Part D plan. With Extra Help, you can enroll in any benchmark plan with $0 premium.
