Dental infections most often start from an untreated cavity, gum disease, or a cracked tooth that lets bacteria reach the pulp (the soft tissue inside the tooth). A periapical abscess forms at the tooth root; a periodontal abscess forms in the gum next to the tooth. Less commonly, deep restorations, a failed root canal, or trauma let bacteria in.

Antibiotics alone do not cure a dental abscess — drainage and definitive dental treatment (root canal, extraction, or gum procedure) are required. Antibiotics ARE appropriate when infection is spreading (facial swelling, fever, lymph node enlargement), in immunocompromised patients, or when a dental procedure must be delayed. Common choices include amoxicillin or amoxicillin-clavulanate; clindamycin for patients with a penicillin allergy.

Seek emergency care immediately for difficulty breathing or swallowing, swelling spreading to the eye or neck, severe facial swelling that crosses the midline, high fever with confusion, or rapidly worsening pain. These can signal Ludwig's angina, cavernous sinus thrombosis, or systemic spread — all life-threatening complications of an untreated dental infection.

Warm salt-water rinses several times a day, OTC pain relievers (ibuprofen rotated with acetaminophen is often most effective), a cold compress on the cheek for swelling, and avoiding very hot, cold, or hard foods on the affected side. Do not place aspirin directly on the gum — it can chemically burn the tissue. Get to a dentist as soon as possible — at-home measures only buy time.

Amoxicillin most commonly causes GI upset, diarrhea, or rash; amoxicillin-clavulanate adds more GI side effects but covers a broader range of bacteria. Clindamycin carries a notable risk of C. difficile colitis. NSAIDs (ibuprofen) can irritate the stomach or affect kidney function with prolonged use. Pain usually improves dramatically once drainage or definitive dental work is performed.
