Fever, chills, flank pain, nausea, and vomiting suggest upper tract involvement (pyelonephritis) — a more serious infection requiring more aggressive treatment.

Urine culture identifies the specific bacteria and its antibiotic sensitivities — important for selecting the right antibiotic and for confirming treatment success in recurrent or complicated UTIs. For uncomplicated UTIs in otherwise healthy women with classic symptoms, empirical treatment without culture is acceptable in many clinical settings, though culture improves accuracy.

Asymptomatic bacteriuria is the presence of bacteria in urine without symptoms — common in older adults, pregnant women, and catheterized patients. It does not require antibiotic treatment in most non-pregnant adults — treating it does not prevent symptomatic UTI and drives antibiotic resistance. Only symptomatic UTI warrants treatment in most cases.

Recurrent UTI risk factors include sexual activity patterns, postmenopausal vaginal atrophy, anatomical factors, constipation, and incomplete bladder emptying. For women with two or more infections in six months, a clinical evaluation identifies contributing factors and enables discussion of preventive strategies including D-mannose, vaginal estrogen, and prophylactic antibiotics.

Trimethoprim-sulfamethoxazole (TMP-SMX) may cause rash and GI upset — sulfa allergy is an important consideration. Nitrofurantoin may cause nausea and is not appropriate for upper tract infections. Fluoroquinolones are reserved for complicated UTIs due to tendinopathy and resistance concerns. Completing the full antibiotic course is essential to prevent recurrence.
