Shingles is caused by reactivation of the varicella-zoster virus — the same virus that causes chickenpox — which lies dormant in nerve tissue after the initial infection. Reactivation is triggered by declining immunity — from aging, stress, illness, or immunosuppression. Anyone who has had chickenpox is at risk.

Shingles itself is not airborne-contagious. However, direct contact with active, fluid-filled blisters can transmit the varicella-zoster virus to people who have never had chickenpox or been vaccinated — causing chickenpox in them, not shingles. Covering the rash until blisters crust over prevents transmission.

Postherpetic neuralgia (PHN) is persistent nerve pain that continues after the rash resolves — lasting months to years in some patients. It's the most serious complication of shingles, occurring in approximately 10–15% of cases overall and more commonly in older adults. Early antiviral treatment reduces its risk.

Shingrix (recombinant zoster vaccine) is recommended for all adults 50 and older — including those who have had shingles previously or received the older Zostavax vaccine. It is approximately 90% effective at preventing shingles and significantly reduces PHN risk even in breakthrough cases.

Antivirals (valacyclovir, famciclovir) are generally very well tolerated — nausea and headache are the most common effects. Corticosteroids, sometimes added for severe cases, carry standard short-course side effects. Shingrix vaccination commonly causes injection site soreness, fatigue, and myalgia for 1–3 days — reflecting a robust immune response.
