Otitis externa is inflammation or infection of the outer ear canal, most often bacterial (commonly Pseudomonas aeruginosa or Staphylococcus aureus). Moisture trapped in the canal — from swimming, showers, humid weather, or sweat — softens the skin and disrupts the protective wax barrier, letting bacteria multiply. Aggressive cleaning with cotton swabs, frequent earbud use, and hearing aids are common contributing triggers.

Otitis externa affects the ear canal (outer ear) — tugging on the earlobe or pressing the tragus reproduces the pain, and the canal itself looks red and swollen. Otitis media affects the middle ear behind the eardrum — pain is deeper, often with fever, and frequently follows a cold. Treatments differ: external typically needs topical ear drops; middle ear infections sometimes require oral antibiotics.

Most cases respond to medicated ear drops — combinations of antibiotic plus steroid (such as ciprofloxacin-dexamethasone or ofloxacin) used for 7–10 days. Acidifying or drying drops may be added. Oral antibiotics are reserved for severe or spreading infection, immunocompromised patients, or suspected malignant otitis externa. Pain control with NSAIDs is often important during the first few days.

Dry your ears thoroughly after swimming or showering (tilt your head, towel gently, use a hair dryer on low at a safe distance). Avoid cotton swabs inside the canal. If you're prone to recurrences, preventive drops (over-the-counter, or a homemade mix of equal parts white vinegar and rubbing alcohol) used right after water exposure can help. Custom or OTC earplugs while swimming reduce risk for frequent swimmers.

Topical antibiotic-steroid drops are generally very well tolerated — occasional local irritation, brief stinging, or transient hearing changes that resolve when treatment ends. Patients with a perforated eardrum should avoid certain ototoxic drops (neomycin- or polymyxin-containing combinations); fluoroquinolone-based drops are preferred in that setting. Oral antibiotics, when needed, carry the usual GI side-effect profile (nausea, diarrhea, occasional rash).
