Tinea versicolor is caused by overgrowth of Malassezia, a yeast that normally lives on the skin. Hot and humid weather, sweating, oily skin, and a weakened immune system can trigger the yeast to multiply and produce substances that interfere with skin pigment. Despite the name, it is NOT a contagious ringworm-style infection — you cannot catch it from someone else.

Tinea versicolor classically produces sharply outlined, scaly patches that are lighter or darker than the surrounding skin and most often appear on the chest, back, shoulders, and neck. It can resemble vitiligo (no scale, complete pigment loss), pityriasis alba (slightly scaly, often on the face in children), eczema (more itch and inflammation), or post-inflammatory pigment changes. A clinician can usually distinguish by appearance and, if needed, a quick KOH skin scraping.

First-line treatments are topical antifungals: selenium sulfide 2.5% lotion or shampoo, ketoconazole 2% shampoo, or ciclopirox cream applied for 1–2 weeks. For widespread, recurrent, or treatment-resistant cases, oral antifungals (fluconazole or itraconazole, typically as short courses) may be prescribed. Maintenance therapy — using a medicated shampoo once or twice a month — is often added when recurrences are frequent.

The yeast and scale resolve within a couple of weeks of effective treatment, but the pigment changes can take weeks to months to even out — especially after sun exposure that has tanned the surrounding skin. Persistence of pale or dark spots does NOT mean treatment failed. Avoid scratching and limit sun exposure during recovery so pigment differences are not amplified.

Topical antifungals are generally very well tolerated — occasional dryness, mild stinging, or scalp and skin irritation are the main effects. Selenium sulfide has a distinctive smell and can stain jewelry. Oral antifungals (fluconazole, itraconazole) are usually well tolerated for short courses but can cause GI upset, headache, or — rarely — liver enzyme changes that warrant monitoring with prolonged use.
